The USMC CAS (Corpsman Assault System) Medical Sustainment Bag is the second primary component of the Marine Corps' four-bag modular Corpsman load carriage system — the larger, extended-mission bag designed to carry the Corpsman's resupply stock, patient care consumables, and sustainment-level medical equipment beyond what fits in the immediate-access Assault Pouch.

Where the Medical Assault Pouch is built for the contact moment — fast access to hemorrhage control and airway gear — the Sustainment Bag is built for what comes after. Per official NSN documentation, it functions as a resupply bag for medical equipment and consumables and can serve as the primary medical bag for mounted operations or patient care in static positions. It carries more, organizes deeper, and gives the Corpsman the capacity to operate through a mass casualty event, a sustained patrol, or a CCP (Casualty Collection Point) without running dry.

The interior is an open compartment fully lined with loop pile fabric for hook-and-loop attachment of the included modular inserts — stacked pocket panels, elastic panels, large and medium reversible pouches, NARC pouch, and medical thigh rig can all be repositioned and swapped to match the Corpsman's personal kit layout and mission requirements. Adjustable shoulder straps, cinch straps, and a carry handle allow it to be worn standalone as a standalone shoulder bag or attached to the FILBE main pack. Exterior loop field for unit ID and patches.
